For me, it usually happens near Rook's Rest. It might be that it's in close proximity to wherever Stannis is located. Hope that helps.

Assassins, actually. As of the recent version, very unbalanced - as negative traits for them arent in.
Yes assassins makes better sense.
So overusing them will give you negative traits? Or will eventually?
You could also tie something to the missions that gives settlement unrest or something similar.
That'd be good. Or, though I have no idea if its possible with the game layout, could it be possible for every use, to have, say, a year (4 turns) marked off his life span?
No, but if used x times you could kill_character.
I would like there to be two shadows spawned and that's it, until the books tell us different. Personally, I want the game to be as close to the books as possible.

I'm pretty sure someone said they are going to make another version more canon later, but isn't it rather impossible to reenact the book right now? Realisticly, if the series DID go on for say, 12 years, you would see a lot more shadows.
No, not really. There are FOUR books thus far. If something hasn't been seen in the story yet, or if something has happened only a few times, I'd prefer to keep it to that number in the mod. If something changes after the other books are released, then in an updated version, we can take the proper steps to make it as true to the books as humanly possible.

But then you have to change the turns per year to reflect the fact that you want to stay as close to the book as possible. You'll also have to disable marriages and the birth of new children to your generals.
As it is now, you are able to play far beyond the time the books cover. And like Kiba pointed out, I think there will be many more shadows during the coming years. Except of course, Stannis dies or he breaks with Melisandre. But we cannot know that now.
