Can be done, has been done in multiple mods. See, for example, the Hundred Years War mod, which has several navigable rivers, including the Rhone, Seine, and Rhine. See also, for example, the unreleased Vacuus Lux Lucis mod, which has many British rivers, such as the Tweed, navigable.

I believe that the Deus lo Volt mod also has a navigable Danube river.
